Every day, seniors, children, and families across Central Florida go without the care they need, not because help is unavailable, but because no one guides them to it. Constellation Unity Foundation was created to bridge that gap, one person at a time.
Mental Health Referral Network
Finding the right mental health professional shouldn't feel like navigating a maze. We connect each client with trusted professionals matched to their specific needs, regardless of their health insurance plan or coverage, and our work doesn't end with the introduction. Our team helps people navigate Medicaid, food stamps, referrals to community and county resources, and other assistance programs, so paperwork and cost never become the reason someone gives up before they start.
Functional Independence Services
For clients with mental health conditions that affect daily functioning, our Functional Independence Specialist helps build and strengthen the skills needed for independent living. That includes daily routines, symptom management, communication, problem solving, and connecting with community resources. Loneliness is one of the most common barriers older adults face, and this program exists to help people build a full, connected life again, not just manage symptoms.
Case Management (TCM)
Our Targeted Case Management case manager assesses each client's full range of needs, builds an individual care plan, and coordinates access to the medical, mental health, social, educational, and community services that support stability and independence. The case manager doesn't provide direct treatment. Instead, they make sure the right services reach the client on time, and they follow up to keep care on track, acting as the connection point between the client, their family, providers, and community resources.
Community Transportation
We provide safe transportation to medical appointments, the grocery store, the pharmacy, and community activities. This reduces isolation, keeps people on track with their care, and supports independent living.
Programs for Children and Youth
We offer tutoring and academic support, recreational activities like parks, movies, and cultural events, and education in social and emotional skills. These programs strengthen academic performance, self esteem, and social connection for the young people we serve.
How We Measure Success
We don't just count the people we serve. We track whether they get better. In 2024, Constellation Unity Foundation served 215 individuals and supported approximately 550 scheduled appointments.
